Namakarana is the fourth of the sixteen Sanskaras — the lifecycle rites that accompany a Hindu from conception to the last rites. Traditionally on the 11th day after birth; we welcome later dates too, especially when family travels in.
Based on the date, time and place of birth, the pujari determines the child's Nakshatra (lunar mansion) and the sacred starting syllables for the name. You choose the name yourself — the pujari blesses it, whispers it into the child's right ear, and enters it into the family register.
